Expedite your visit by completing new patient registration before you arrive. 

How to prepare for your first visit?

Please arrive 15 minutes earlier than your scheduled appointment time to fill out registration forms. Or, save time and fill out online!

Remember to bring:

  • Driver's License or Valid ID

  • Insurance Card

  • Patient Registration Forms

  • Your referral letter and any reports, X-rays or other imaging if applicable.

  • Current medication list

What to expect during your visit 

​

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
